揿针联合柴芍六君汤治疗肝郁脾虚型代谢相关性脂肪肝42例
Ringheaded Thumbtack Needle Combined with Chaishao Liujun Decoction in Treatment of Metabolic Dysfunction-Associated Fatty Liver with Stagnation of Liver Qi and Spleen Deficiency Type in 42 Cases

Abstract
Objective:To observe the clinical effect of ringheaded thumbtack needle combined with Chaishao Liujun decoction for metabolic dysfunction-associated fatty liver(MAFL)with stagnation of liver Qi and spleen deficiency type.Methods:A total of 84 MAFL patients with stagnation of liver Qi and spleen deficiency type who visited the department of spleen and stomach diseases at Zhangzhou Traditional Chinese Medicine Hospital Affiliated to Fujian University of Traditional Chinese Medicine from July 2023 to February 2025 were selected and randomly divided into control group and observation group,with 42 patients in each group,using a random number table method.Both groups were given routine dietary control and exercise.The control group was treated with oral administration of polyene phosphatidylcholine capsules,while the observation group was treated with ringheaded thumbtack needle combined with oral administration of Chaishao Liujun decoction.Both groups were treated continuously for 2 months.Body mass and height were measured before and after treatment in two groups,and the body mass index(BMI)was calculated;the FibroScan liver instantaneous elasticity analyzer was applied to detect two groups of controlled attenuation parameters(CAP)before and after treatment;the TCM syndrome scores and serum lipid metabolism indicators such as total cholesterol(TC)and triglycerides(TG),liver function indicators such as alanine aminotransferase(ALT)and aspartate aminotransferase(AST),as well as the levels of superoxide dismutase(SOD),tumor necrosis factor-α(TNF-α),and interleukin-1β(IL-1β)were measured before and after treatment in two groups;the therapeutic effects of two groups were compared,and the occurrence of adverse reactions was observed.Results:Compared with those before treatment,the levels of CAP and serum TC,ALT,AST,TNF-α,IL-1β in both groups significantly re-duced after treatment(P<0.05),while the level of serum SOD significantly increased(P<0.05);the TCM syndrome score,BMI,and the levels of serum TG in the observation group significantly decreased after treatment(P<0.05).Compared with the control group,the BMI,CAP,and the levels of serum TC,TG,ALT,TNF-α,IL-1β in the observation group significantly decreased after treatment(P<0.05),and the level of serum SOD significantly increased(P<0.05).The total effective rate of the observation group was 90.48%,significantly higher than the 73.81%of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ringheaded thumbtack needle combined with Chaishao Liujun decoction can improve the clinical symptoms of MAFL with stagnation of liver Qi and spleen deficiency type,correct lipid metabo-lism disorder,reduce liver fat accumulation,alleviate oxidative stress and inflammatory damage,and thereby improve liver function.关键词
