Current status and influencing factors of social alienation among head and neck cancer survivors

Abstract
Objective:With advances in treatment technology,the number of head and neck cancer survivors has been increasing.However,this group is at high risk for social isolation due to appearance changes and physiological impairments caused by the disease and its treatment.Social isolation,a negative psychological state characterized by a subjective sense of being alienated and excluded,is closely associated with poor cancer prognosis and increased risk of mental disorders.Therefore,this study aims to investigate the current status of social isolation among head and neck cancer survivors and explore its influencing factors. Methods:Using a convenience sampling method,head and neck cancer survivors(pathologically confirmed and having completed primary radical treatment)who visited the Affiliated Central Hospital of Jiangnan University between August 2023 and June 2024 were recruited.Participants completed face-to-face questionnaire surveys.General demographic data were collected through a self-designed general information form.The General Alienation Scale(GAS),Social Impact Scale(SIS),Distress Temperature(DT),and Cancer Loneliness Scale(CLS)were used to assess levels of social alienation,stigma,psychological distress,and loneliness,respectively. Results:A total of 138 head and neck cancer survivors were included.The total GAS score,SIS score,and CLS score were 33.00(30.00,37.50),54.90±8.31,and 16.00(13.00,20.00),respectively;the DT score was 3.00(1.00,6.00).The total GAS score and its subdimension scores were positively correlated with SIS score,CLS score,and DT score(all P<0.01).Multiple linear regression analysis identified marital status(non-married),treatment phase(follow-up or maintenance therapy),SIS score,and DT score as significant influencing factors of GAS score(all P<0.05),explaining 65.4%of the total variation(R2=0.654). Conclusion:Social isolation among head and neck cancer survivors is at a moderate level.Healthcare providers should pay special attention to widowed survivors,those in the maintenance therapy phase,and individuals with high levels of stigma or psychological distress.Targeted interventions based on these influencing factors should be developed to reduce self-closure and social avoidance among head and neck cancer survivors.关键词
