Clinical efficacy of Tongnao Yiqi formula in the treatment of acute ischemic stroke of the wind-phlegm obstructing collaterals type and its effect on coagulation function

Abstract
Objective:Conventional Western medical therapy for acute ischemic stroke(AIS)often yields suboptimal outcomes,whereas traditional Chinese medicine(TCM)has shown remarkable efficacy in recent years.This study aims to evaluate the therapeutic effect of Tongnao Yiqi formula on patients with AIS of the wind-phlegm obstructing collaterals type,and to provide clinical reference for AIS management. Methods:Clinical data of patients with wind-phlegm obstructing collaterals type AIS treated in the Department of Brain Disease,Nanjing Integrated Traditional Chinese and Western Medicine Hospital from January 2024 to December 2024,were retrospectively analyzed.A total of 50 patients who received standard treatment were assigned to the control group,while another 50 who received standard treatment combined with Tongnao Yiqi formula comprised the treatment group.Clinical efficacy was compared between the 2 groups.Thrombotic indicators,thrombin-antithrombin complex(TAT),plasminogen-α2 plasmin inhibitor complex(PIC),thrombomodulin(TM),and tissue plasminogen activator-plasminogen activator inhibitor complex(t-PAIC),and coagulation parameters,activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT),and fibrinogen(FIB),were measured before and after 2 weeks of treatment.Adverse events during treatment were also recorded. Results:After 2 weeks of treatment,the total effective rate in the treatment group was significantly higher than that in the control group(P<0.05).TCM syndrome scores in both groups decreased compared with baseline,with greater reductions in the treatment group(all P<0.05).Levels of TAT,PIC,TM,and t-PAIC decreased in both groups,and were significantly lower in the treatment group than those in the control group(all P<0.05).APTT,PT,and TT increased in both groups after treatment,with greater increases in the treatment group(all P<0.05),whereas FIB levels decreased in both groups and were lower in the treatment group(all P<0.05).The total incidence of adverse reactions did not differ significantly between the 2 groups(P>0.05). Conclusion:Tongnao Yiqi formula significantly improves clinical efficacy and reduces thrombus formation in patients with AIS of the wind-phlegm obstructing collaterals type,and with good safety and tolerability.关键词
