Abstract
Congenital cutaneous candidiasis(CCC)is a rare neonatal intrauterine infectious disease primarily caused by Candida albicans,characterized by diffuse skin rash and,in severe cases,may progress to disseminated infection or even death.This paper reports a case of an extremely preterm infant,one of twins,diagnosed with CCC induced by Candida albicans.A pregnant woman with twin gestation conceived via in vitro fertilization-embryo transfer and concomitant vulvovaginal candidiasis was admitted to the Department of Obstetrics and Gynecology,Third Xiangya Hospital,Central South University,on August 15,2024.She experienced preterm labor at 27+2 weeks of gestation,giving birth to a female and a male infant.The female neonate presented with generalized erythematous rash and local skin erosions at birth.Pathogenetic examination confirmed Candida albicans infection,establishing the diagnosis of CCC.The male twin showed no cutaneous lesions and tested negative for fungal infection.The infection route in this case was considered to be ascending vaginal infection resulting in intra-amniotic infection,rather than vertical transplacental transmission.The affected infant received topical and intravenous fluconazole therapy,leading to resolution of symptoms and favorable prognosis.This case highlights that vulvovaginal candidiasis during pregnancy is a major risk factor for intrauterine infection.Candida albicans can ascend from the vagina to invade the amniotic cavity and infect the fetus.Therefore,clinical management should strengthen screening and standardized treatment of candidiasis during pregnancy,particularly in women with assisted reproduction(in vitro fertilization-embryo transfer),cervical cerclage,or high risk of preterm delivery,to prevent CCC and related complications through timely infection control and intervention.关键词
