Clinical efficacy of robot-assisted"Pocket"pancreaticojejunostomy(with surgical video)
摘要
Abstract
Objective:To investigate the clinical efficacy of a modified pancreaticojejunal anastomosis technique in robot-assisted pancreaticoduodenectomy(RPD).Methods:101 patients who underwent RPD at the First Affiliated Hospital of Naval Medical University from March 2015 to October 2024 were enrolled.Among them,51 patients who received"HO"half-purse-string binding pancreaticojejunostomy were assigned to the HO group,and 50 patients who received"Pocket"pancreaticojejunostomy were assigned to the Pocket group.Perioperative clinical data were systematically collected and compared between the two groups of patients.Results:No statistically significant differences were observed between the two groups in postoperative hemoglobin levels,nutritional indicators,liver function indicators,heart rate fluctuations,abdominal drainage volume,or length of hospital stay(P>0.05).The average anastomosis time in the Pocket group was significantly shorter than that in the HO group.No cases of pancreatic fistula occurred in the Pocket group,while 6 cases developed in the HO group and all recovered and discharged after continuous drainage and pancreatic enzyme inhibition therapy,with no mortality in either group.Conclusions:The novel"Pocket"pancreaticojejunostomy has advantages of easy performance,reduced time consumption,and lower pancreatic fistula risk in RPD.It not only significantly enhances surgical safety but may also mitigate long-term pancreaticojejunal anastomotic stenosis,which has promising clinical application prospects.关键词
