Clinical application of bipolar electrocoagulation in lymph node dissection during robot-assisted radical gastrectomy(with surgical video)

Abstract
Objective:To evaluate the feasibility and potential advantages of bipolar electrocoagulation in lymph node dissection during robot-assisted radical gastrectomy for gastric cancer.Methods:Clinical data were retrospectively analyzed from 60 patients who underwent robot-assisted radical gastrectomy at the Affiliated Hospital of Xuzhou Medical University between January 2020 and January 2024.Bipolar electrocoagulation was employed throughout the procedures for surgical field expansion,tissue dissection,lymph node dissection,and vascular sealing.Results:The median age of patients was 62 years,with a median BMI of 23.3 kg/m2.The median preoperative preparation time was 26.3 minutes,with an average operative time of 236 minutes and an average intraoperative blood loss of 36 mL.The average time for suprapancreatic lymph node dissection was 35 minutes,with an average of 14.74 lymph nodes dissected at this site and a total of 31.12 lymph nodes dissected on average.No severe intraoperative complications occurred.The average time to first flatus was 2.52 days,the average time to first oral intake was 3.56 days,the median postoperative length of hospital stay was 7.6 days,the average cost of robot-assisted surgery was CNY 22,200,and the average total hospitalization cost was CNY 85,300.Postoperative complications occurred in 5 cases(8.3%),and all patients recovered after treatment.Conclusion:Bipolar electrocoagulation is safe and effective in robot-assisted radical gastrectomy for gastric cancer.It can facilitate precise intraoperative hemostasis,high-precision dissection and lymph node dissection,and provide an excellent surgical field.Combined with other advantages of the robotic surgical system,it can enhance the overall surgical safety and precision.关键词
