Abstract

Objective:To explore the application value of the TruScreen AI system in cervical cancer screening and diagnosis,and evaluate its potential in improving diagnostic accuracy,reducing misdiagnosis rate and enhancing screening efficiency.Methods:A retrospective cohort study was made on 306 patients who underwent cervical cancer screening at Nanjing Tongren Hospital from April 2024 to April 2025.All included patients underwent TruScreen AI system evaluation,human papillomavirus(HPV)testing,and liquid-based thin-layer cytology(TCT)testing.When single or positive results were detected,colposcopy was performed,and pathological biopsy was conducted on suspected cases.Taking pathological biopsy results as the gold standard,the Kappa index was used to evaluate the diagnostic consistency of TruScreen AI system-assisted evaluation alone and in combination with HPV and TCT testing.Further analysis was performed on the specificity,sensitivity,and accuracy of TruScreen AI system-assisted evaluation.The receiver operating characteristic(ROC)curve was used to explore its diagnostic area under the curve(AUC).Results:The positive rates of TruScreen AI system were highest in the age groups of 40-<50 years and 50-<60 years,at 45.08%and 48.39%respectively.There were statistically significant differences in screening results among different age groups(P<0.05).The positive rates of TruScreen AI system,HPV,and TCT testing increased significantly with the severity of pathological findings,with both TruScreen AI system and HPV testing showing positive rates exceeding 90%in HSIL and above lesions,while TCT testing reached 93.75%.All three methods showed significant correlation with pathological results(P<0.05).The Kappa consistency values among TruScreen AI system,HPV testing,TCT testing,TruScreen AI system+HPV testing,and TruScreen AI system+TCT testing with pathological examination were 0.770,0.376,0.547,0.355,and 0.442,respectively.The standalone diagnostic sensitivity of TruScreen AI system reached 98.2%with a specificity of 73.1%.When combined with HPV or TCT testing,the sensitivity increased to 99.1%,though specificity decreased.The AUC of TruScreen AI system screening was 0.851,significantly higher than that of HPV(0.680)and TCT(0.749)testing,indicating superior diagnostic performance in cervical cancer screening(P<0.05).The AUC values for TruScreen AI system+HPV testing and TruScreen AI system+TCT testing were 0.919 and 0.961,respectively,significantly higher than standalone predictions.Conclusion:The TruScreen AI system-assisted diagnostic system has high diagnostic efficacy in cervical cancer screening,with better diagnostic accuracy compared to conventional HPV and TCT tests.It can significantly improve screening accuracy when used alone or in combination with other methods.
