基于紧密型城市医联体的"互联网+慢性伤口护理"管理模式的构建与应用
Construction and application of"Internet+chronic wound care"management model based on the compact urban medical consortium

Abstract
Objective:Based on the construction of the compact urban medical consortium,this study innovatively established a hospital-community-family ternary linkage"Internet+chronic wound care"management system,and systematically evaluated its clinical application effect.Methods:A hospital-community-family three-level linkage team was established to clarify responsibilities and formulate systems,connect with the"Internet hospital"platform,train community medical staff and family caregivers in wound management skills,and form a management model based on"Internet+chronic wound care".A total of 110 patients with chronic wounds in Huzhou urban medical consortium from January 2020 to June 2023 were selected as the observation objects by using the convenient sampling method.The wound Pressure Ulcer Scale for Healing(PUSH)score,quality of life score and caregiver burden score were evaluated in the baseline period,the first week and the second week of intervention,respectively.The nursing satisfaction of patients were evaluated at baseline and the second week of intervention.At the same time,the professional theoretical knowledge and operational skills of 50 community nurses were longitudinally compared and analyzed.Results:There were statistically significant differences in the PUSH score,overall quality of life score,functional dimension score,symptom dimension score and caregiver burden score of patients at different time points(P<0.001).The overall quality of life score and the score of the functional dimension showed an upward trend over time,while the PUSH score,the score of the symptom dimension and the score of the caregiver burden showed a downward trend over time.Patient satisfaction increased from 61.82%to 92.73%(P<0.001),and the pass rate of community nurses'assessment rose from 28.00%to 72.00%.Both patient satisfaction and the pass rate of community nurses'assessment showed significant improvements(P<0.001).Conclusions:The"Internet+nursing"ternary linkage chronic wound management system constructed in this study improves the chronic wound nursing service system,realizes hierarchical management,and significantly improves the quality of life of patients.Through standardized management methods,this model not only improves the ability of community medical staff to manage chronic wounds,but also reduces the physical and mental burden of patients and their families,and greatly improves patient satisfaction.This innovative management model provides new ideas and practical paths for the standardized management of chronic wounds.关键词
