Abstract

Objective To explore the relationship between height growth and deformity progression in female patients with adolescent idiopathic scoliosis(AIS),and investigate the predictive value of Risser grading and menarche timing for deformity progression risk.Methods A retrospective analysis was performed on the clinical data of 335 female patients with AIS.Their menarche timing and skeletal maturity index(Risser grading)were recorded.The height growth rate and AIS-related deformity progression rate(expressed as Cobb angle change rate)were compared among patients with different Risser grades.The Kappa coefficient was then used to assess the consistency between deformity progression and growth and development status.Additionally,a linear regression model and the area under the receiver operating characteristic curve(AUC)were used to evaluate the influencing factors of deformity progression and the predictive value of these factors for the progression peak of AIS-related deformity.Results The deformity progression rate of patients with Risser grade 1 was significantly higher than that of those with Risser grades 0,2,and 3(P<0.05).The height growth rate of patients with Risser grade 0 was significantly higher than that of patients with other Risser grades(P<0.01).The Kappa coefficient for the consistency between height growth peak and deformity progression was 0.056(P=0.346).Risser grading was an important factor influencing deformity progression(P<0.05).Menarche timing exhibited a trend of affecting deformity progression(B=-0.023,P=0.069).The combination of Risser grading and menarche timing had a certain predictive value for the risk of AIS-related deformity progression(AUC=0.709,P=0.019).Conclusion In female patients with AIS,the peak of deformity progression occurs later than the peak of height growth.While the Risser grading and menarche timing exhibit poor predictive ability for the height growth peak,they hold certain predictive value for AIS-related deformity progression.
