孤独症谱系障碍儿童首诊年龄与临床表现和神经发育水平的关系
Associations between age at initial diagnosis and clinical manifestations,neurodevelopmental level in children with autism spectrum disorder

Abstract
Objective To investigate the associations between age at initial diagnosis and clinical manifestations and neurodevelopmental level in children with autism spectrum disorder(ASD).Methods A total of 718 children with ASD who received their initial diagnosis between January 2022 and December 2023 were enrolled in this study.They were grouped based on their age at initial diagnosis:24 to 47 months,48 to 71 months,and 72 to 98 months.Assessments were conducted using the autism behavior checklist(ABC),childhood autism rating scale(CARS),and Gesell developmental schedules(GDS).The relevant data were statistically analyzed.Results No statistically significant differences were observed in the total CARS score or the GDS language development quotient among ASD children in different initial diagnosis age groups(all P>0.05).In contrast,statistically significant differences were found in the total ABC score and the GDS gross motor,fine motor,adaptive ability,and social ability developmental quotients(all P<0.05).Specifically,the total ABC score and the developmental quotients for gross motor,fine motor,adaptive ability,and social ability in the 48 to 71 months group were significantly lower than those in the 24 to 47 months group(all P<0.05).Pearson correlation analysis revealed that age at initial diagnosis was significantly negatively correlated with total ABC score(r=-0.086),total CARS score(r=-1.117),and GDS adaptive ability developmental quotient(r=-0.184),gross motor(r=-0.245),fine motor(r=-0.154),and social ability developmental quotient(r=-0.172)(all P<0.05 or P<0.01),but showed no significant correlation with GDS language quotient(P>0.05).Conclusion Children with ASD who present with atypical clinical manifestations but show delays in multiple developmental quotients as assessed by the GDS are often diagnosed later.It is crucial to strengthen the popularization of ASD-related knowledge,improve screening capabilities,and enhance specialized diagnostic levels in primary clinics,so as to minimize the impact of ASD on children's development.关键词
