Abstract

Objective:Investigating postoperative serum levels of CD40 ligand(CD40L),monocyte chemotactic pro-tein-1(MCP-1),and carcinoembryonic antigen(CEA)and relationship with infection in patients with oral squamous cell car-cinoma(OSCC).Methods:From January 2020 to January 2025,195 OSCC patients who underwent surgical treatment in our hospital were included.The serum CD40L,MCP 1,and CEA were measured.General clinical data were collected and ana-lyzed.Logistic regression was used to explore the influencing factors of postoperative infection in OSCC patients.Receiver op-erating characteristic(ROC)curves were plotted to explore the predictive value of serum CD40L,MCP1,and CEA for postop-erative infection.Results:Postoperative serum CD40L in OSCC patients was clearly increased,while MCP1 and CEA were clearly decreased(P<0.05).The infected group had clearly lower serum CD40L,and clearly higher MCP1 and CEA than the uninfected group(P<0.05),while the differences in tumor staging,differentiation degree,and tracheotomy between two groups were statistically clear(P<0.05).Tumor stage Ⅲ~Ⅳ,low differentiation degree,tracheotomy,and high MCP1 and CEA were all risk factors affecting postoperative infection in OSCC patients,while high CD40L was a protective factor affect-ing postoperative infection in OSCC patients(P<0.05).The area under curve(AUC)values of serum CD40L,MCP1,CEA,and their joint in predicting postoperative infection were 0.840,0.858,0.808,and 0.957,respectively.The AUC predicted by the joint was clearly higher than that predicted by CD40L alone(Z=4.317,P<0.001),MCP1 alone(Z=4.183,P<0.001),and CEA alone(Z=4.367,P<0.001).Conclusion:Serum CD40L is decreased,and MCP 1 and CEA are increased in OSCC patients.Tumor staging,differentiation degree,tracheotomy,and serum CD40L,MCP1,and CEA are all influencing factors for postoperative infection in OSCC patients.
