2型糖尿病患者糖化血红蛋白动态变异性、胰岛素抵抗指数与糖尿病周围神经病变风险的关系
Relationship Between Glycated Hemoglobin Temporal Variability,Insulin Resistance Index,and Risk of Diabetic Peripheral Neuropathy in Type 2 Diabetes Patients

Abstract
Objective This study aimed to investigate the influence of dynamic variability of glycosylated hemoglobin(HbAlc),insulin resistance index(HOMA-IR),and the risk of peripheral neuropathy(DPN)in patients with type 2 diabetes melli-tus(T2DM).Methods A total of 79 patients with T2DM complicated by DPN treated at Henan Provincial Third People's Hospital from January 2021 to May 2022 were selected as the DPN group.A control group of 79 patients with T2DM alone was matched 1∶1 from the same period.Clinical data from the 2 years prior to admission were retrospectively collected for all patients.Mean and stan-dard deviation(SD)values of HbA1c and fasting blood glucose(FBG)served as indicators of dynamic variability.Differences in clinical characteristics,HbA1c dynamic variability,and HOMA-IR were compared between groups.Spearman correlation analysis examined the relationship between HbA1c dynamic variability,HOMA-IR,and DPN risk.Binary logistic regression further identi-fied risk factors for DPN development in T2DM patients.Result Compared with the control group,patients in the DPN group showed significantly higher levels of age,disease duration,hypertension prevalence,triglycerides(TG),total cholesterol(TC),FBG-mean,FBG-SD,HbA1c-mean,HbA1c-SD,and HOMA-IR(P<0.05).Spearman correlation analysis revealed positive corre-lations between FBG-mean,FBG-SD,HbA1c-mean,HbA1c-SD,and HOMA-IR with DPN risk in T2DM patients(P<0.05).Using the occurrence of DPN as the dependent variable and incorporating variables that showed statistical significance in univariate analysis as independent variables,binary logistic regression analysis indicated that advanced age,high TC,high HbA1c-SD,and high HOMA-IR were independent risk factors for DPN in T2DM patients(P<0.05).Conclusion Advanced age,high TC,high HbA1c-SD,and high HOMA-IR were significantly associated with the occurrence of DPN in patients with T2DM.关键词
