脓毒症休克患者血清miR-214、miR-34b和miR-22表达与病情严重程度及预后的相关性
Association of Serum miR-214,miR-34b,and miR-22 Levels with Disease Severity and Prognosis in Septic Shock Patients

Abstract
Objective This study aimed to investigate and analyze the expression levels of serum microribonucleic acid(miR-214,miR-34b,and miR-22)in patients with septic shock,and to analyze the relationship between the three and the severity of dis-ease and prognosis of patients.Methods From January 2020 to January 2024,a total of 128 septic shock patients admitted to Nan-yang Central Hospital were included as the septic shock group.According to the prognosis,they were separated into a survival group(n=67)and a death group(n=61).Another 130 non septic shock patients who received treatment in our hospital were included as the septic no-shock group.QRT-PCR method was applied to detect the expression levels of miR-214,miR-34b,and miR-22.Pear-son method was applied to analyze the correlation between serum miR-214,miR-34b,miR-22 with Acute Physiology And Chronic Health Evaluation(APACHEⅡ)score,and Sequential Organ Failure Assessment(SOFA)score.Multivariate logistic regression was applied to analyze the influencing factors of prognosis.Receiver operating characteristic(ROC)curves were plotted to analyze the pre-dictive value of serum miR-214,miR-34b,and miR-22 for prognosis.Results Compared with the non-shock group,the shock group showed significantly decreased serum expression levels of miR-214 and miR-22,but increased expression of miR-34b(P<0.05).MiR-214 and miR-22 were negatively correlated with APACHEⅡ score,and SOFA score(P<0.05),while miR-34b was positively correlated with APACHEⅡ score,and SOFA score(P<0.05).SOFA score,shock index,and serum miR-34b were all factors affecting mortality(P<0.05),while miR-214 and miR-22 were protective factors(P<0.05).ROC curve results showed that miR-214,miR-34b,miR-22,and their combined AUC for predicting mortality were 0.838,0.812,0.825,and 0.922,respectively.Combined prediction demonstrated superior performance compared to individual predictions.Conclusion In patients with septic shock,serum levels of miR-214 and miR-22 were downregulated while miR-34b was upregulated,with their expression patterns showing significant correla-tions with disease severity.The combined detection of these three miRNAs demonstrated high predictive value for patient prognosis,suggesting their potential as novel molecular biomarkers for clinical assessment of disease status and outcome prediction.关键词
