从"伏风脑络"病机论治多发性硬化
Treating multiple sclerosis based on the pathogenesis of"Latent Wind in Brain Collaterals"

Abstract
Based on the traditional Chinese medicine(TCM)theory of"Latent Wind"(Fufeng)and the pathological characteristics of brain collateral damage,this study proposes the pathogenesis of"Latent Wind in Brain Collaterals"(Fufeng Naoluo).Latent Wind resides in the brain collaterals and Xuanfu(subtle portals),exhibiting the wind pathogen's"migratory volatility"and the latent pathogen's"concealment awaiting activation."This leads to the pathological outcome of"collateral damage and marrow depletion"(Luosun Suixiao),which aligns with the clinical features of multiple sclerosis(MS),including sudden vision loss,limb weakness and paresthesia,and relapsing-remitting progression.We further establish a pathogenesis framework for MS-induced disability:latent wind in brain collaterals disturbs the sea of marrow(Suihai)with its uprising nature,impairs the blood-brain barrier through its dispersing nature,and spreads along the Governor Vessel(Dumai)due to its migratory nature,ultimately resulting in malnourished marrow and dysfunctional neural mechanisms.Targeting this core mechanism,we emphasize wind-dispelling herbs to"unblocking the cerebral portals"(Jie Naogai),as"failure to eliminate latent wind precludes the opening of Xuanfu."Supporting healthy Qi and eliminating pathogens are integral throughout treatment.A proven case by Academician WANG Yongyan is included,offering a novel perspective for TCM treatment of multiple sclerosis.关键词
