摘要

Abstract

Objective:To explore the impact of serum 25-hydroxyvitamin D(25(OH)D)levels and Vitamin D receptor(VDR)gene polymorphisms(BsmI,FokI,ApaI,TaqI)on the occurrence of gestational diabetes mellitus(GDM)in pregnant women from high-altitude regions ofKunming, and to assess their potential as predictive factors. Methods:A prospective study was conducted on 57 pregnant women undergoing prenatal examinations between June 2019 and June 2020,divided into GDM group(25 cases),insulin-requiring GDM(GDM-RI)group(6 ca-ses),and normal pregnancy group(26 cases).Serum 25(OH)D levels, homeostasis model as-sessment of insulin resistance(HOMA-IR),and VDR gene polymorphisms were measured and analyzed for their associations with GDM.Results:The 25(OH)D levels in the GDM and GDM-RI groups were significantly lower than those in the normal group(P<0.05),and negatively correlated with HOMA-IR(r=-0.393,P=0.002).The B allele frequency of the VDR BsmI polymorphism was higher in the GDM-RI group(P<0.05).Multivariate logistic regression anal-ysis indicated that only 25(OH)D was an independent predictor of GDM(OR=0.184,95%CI:0.053~0.647).Conclusion:In high-altitude regions of Kunming,Vitamin D deficiency may increase GDM risk by exacerbating insulin resistance, while VDR BsmI polymorphism may be involved in disease progression.Further studies with larger sample sizes are needed to validate these findings.
