Assessment of placental volume in early pregnancy:three-dimensional ultrasonography vs magnetic resonance imaging

Abstract
Objective:To evaluate the accuracy of placental volume measurements in early pregnancy using three-dimensional ultrasound(3D-US)and magnetic resonance imaging(MRI),with the water displacement method serving as the gold standard.Methods:A total of 45 singleton pregnancies terminated at 11~13+6 weeks of gestation were included.Placental volumes were measured using 3D-US(VOCAL method with 30° rotation)and MRI(manual segmentation with ITK-SNAP software).The actual placental volume was determined using the water displacement method.Pearson correlation analysis was performed to assess the association between 3D-US,MRI,and actual placental volumes.Bland-Altman analysis was used to evaluate the bias and agreement of 3D-US and MRI measurements compared to the gold standard.Re-sults:Both 3D-US and MRI underestimated the actual placental volume[(45.71±13.59)cm3,(47.07±13.15)cm3 vs(47.78±15.63)cm3].Both methods showed significant correlation with the actual volume[3D-US:r=0.85,P<0.001;MRI:r=0.95,P<0.001].MRI demonstra-ted smaller bias(-0.89cm3 vs-2.07cm3)and narrower limits of agreement(-11.21~9.43 cm3 vs-18.24~14.10cm3)compared with 3D-US.Conclusion:Three-dimensional ultrasound can accurately measure placental volume in early pregnancy.Although its accuracy is slightly inferior to MRI,3D-US remains a convenient,feasible,and reliable imaging modality for placen-tal volumetry,particularly considering its role as the first-line diagnostic tool in obstetric care.关键词
