Abstract
Chronic disease has become a major public health problem that seriously endangers the health of our people.The number of patients with chronic diseases in China is huge and there are multiple diseases coexisting.The traditional medical model is highly dependent on the manual operation of medical care.The existing system only supports single-disease chronic disease management,and the data of each disease are isolated from each other.This paper discusses the construction of chronic disease intelligent follow-up system for multi-disease management following the top-level design concept.Based on multi-modal data fusion,dynamic form,Knowledge Graph and AI voice follow-up technology,the system integrates joint analysis of image,text and biochemical indicator data.The system realizes the functions of supporting multi-disease chronic disease management path,differential follow-up data collection and intelligent follow-up result analysis.The system provides an efficient tool for multi-disease chronic disease follow-up management,breaks through the limitations of traditional single disease single data mode,is compatible with complex chronic disease comorbidity scenarios,has high versatility and high reusability,and meets the needs of doctors to effectively carry out follow-up intervention and patient self-management.关键词
