Correlation analysis of serum homocysteine,interleukin-6,and matrix metalloproteinase-9 with cerebral microbleeds in patients with ischemic stroke

Abstract
Objective To explore the relationship between serum homocysteine(Hcy),interleukin-6(IL-6),matrix metalloproteinase-9(MMP-9),and cerebral microbleeds in patients with ischemic stroke.Methods From January 2021 to June 2023,275 patients with ischemic stroke who visited Hebei Petro China Central Hospital were selected as the research subjects.Based on the occurrence of cerebral microbleeds,the patients were divided into an observation group(118 cases)and a control group(157 cases).General information of patients in the two groups was compared.According to the number of cerebral microbleeds,patients in the observation group were divided into grade 1(41 cases),grade 2(53 cases),and grade 3(24 cases)groups,and serum levels of Hcy,IL-6,and MMP-9 in patients with different severity of cerebral microbleeds were compared.Based on their follow-up,patients in the observation group were divided into a good prognosis group(91 cases)and a poor prognosis group(27 cases)to compare their serum levels of Hcy,IL-6,and MMP-9.Logistic regression was applied to analyze the factors influencing cerebral microbleeds in patients with ischemic stroke.Spearman correlation analysis was applied to analyze the correlation between serum levels of Hcy,IL-6,MMP-9,and the severity of cerebral microbleeds.ROC curve was applied to analyze the prognostic value of serum Hcy,IL-6,and MMP-9 for the prognosis of patients with cerebral microbleeds.Results Age,proportion of hypertension,levels of Hcy,IL-6,and MMP-9 in the observation group were higher than those in the control group,the differences were statistically significant(t/χ2=5.597,6.090,7.852,8.471,9.665;P<0.05).Age,hypertension,Hcy,IL-6,and MMP-9 were factors influencing cerebral microbleeds in patients with ischemic stroke(OR=3.118,3.625,3.786,3.642,3.527;P<0.05).The serum levels of Hcy,IL-6,and MMP-9 in patients in the grade 1,grade 2,and grade 3 groups increased in sequence,with statistically significant differences(F=53.976,78.512,47.013;P<0.05).Serum levels of Hcy,IL-6,and MMP-9 were positively correlated with the severity of cerebral microbleeds(r=0.627,0.713,0.548;P<0.05).The levels of serum Hcy,IL-6,and MMP-9 in the poor prognosis group were higher than those in the good prognosis group,with statistically significant differences(t=7.438,8.320,7.486;P<0.05).The combination of serum Hcy,IL-6,and MMP-9 demonstrated higher AUC value over individual markers for prognostic outcomes in patients with cerebral microbleeds(Z=2.732,2.253,2.217;P<0.05).Conclusion The levels of serum Hcy,IL-6,and MMP-9 are correlated with the severity of cerebral microbleeds in patients with ischemic stroke,and exhibit certain predictive value for the poor prognosis of patients.关键词
