Correlation between peripheral blood novel inflammatory markers and carotid plaque vulnerability in patients with acute ischemic stroke

Abstract
Objective To investigate the correlation between novel inflammatory biomarkers in peripheral blood and carotid plaque vulnerability in patients with acute ischemic stroke(AIS),and assess their predictive value for carotid plaque vulnerability.Methods A total of 146 AIS patients with carotid plaque thickness≥2.5 mm,identified by carotid ultrasonography at the North China University of Science and Technology Affiliated Hospital from March 2022 to October 2023 were selected.Based on superb microvascular imaging(SMI)examination of carotid plaque neovascularization,the patients were divided into a high-grade subgroup(104 cases)and a low-grade subgroup(42 cases).Patients'peripheral blood levels of novel inflammatory biomarkers were collected,including neutrophil-to-lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),monocyte to lymphocyte ratio(MLR),monocyte to high-density lipoprotein cholesterol ratio(MHR),and apolipoprotein B/apolipoprotein A1 ratio(Apo-B/Apo-A1).The differences in novel inflammatory biomarkers levels between the high-grade subgroup and the low-grade subgroup in AIS patients were compared.Logistic regression analysis was employed to identify the factors influencing the grade of neovascularization in carotid plaques.Receiver operating characteristic(ROC)curve analysis was conducted to analyze the diagnostic value of novel inflammatory biomarkers for high-grade subgroup in AIS patients.Results After balancing for gender,age,body mass index,and smoking index using covariance analysis,the levels of novel inflammatory biomarkers as Apo-B/Apo-A1,NLR,and MHR in AIS patients were significantly different between the high-grade and low-grade subgroups(P<0.05).Multivariate Logistic regression analysis showed that Apo-B/Apo-A1,MHR,and NLR were influencing factors of neovascularization grade of carotid plaques in AIS patients.The areas under the ROC curve(AUCs)of ApoB/ApoA1,MHR,NLR,and their combination for predicting high-grade subgroup in AIS patients were 0.776,0.650,0.615,and 0.802,respectively.The AUC of the combination was the largest,the sensitivity and specificity were 85.6%and 71.4%,respectively.Conclusion Apo-B/Apo-A1,NLR,and MHR are factors influencing carotid vulnerable plaques in AIS patients,the combined diagnostic value of these three parameters is relatively high.关键词
