Abstract
Objective To analyze differences in the expression of Brevican and its related fragments between patients with Alzheimer's disease(AD)and patients with vascular dementia(VD).Methods Sixty patients with AD and sixty patients with VD treated at Nanyang Central Hospital from February 2023 to March 2024 were enrolled as the AD group and the VD group,respectively.The severity of leukoaraiosis(white matter rarefaction)and the expression levels of brevican and related indices were compared between the two groups.Receiver operating characteristic(ROC)curve analysis was used to determine the discriminative value of brevican and its related indices for differentiating AD from VD.Results In the VD group,moderate and severe leukoaraiosis were more common,whereas in the AD group mild leukoaraiosis predominated;the difference in leukoaraiosis severity between the two groups was statistically significant(P＜0.05).Compared with the AD group,the VD group showed significantly lower levels of brevican,the N-terminal fragment of brevican(N-Brevican),and the N-Brevican/Brevican-A ratio,while Brevican-A levels were significantly higher(P＜0.05).ROC analysis demonstrated the ability of these markers to distinguish VD from AD:the area under the curve(AUC)for brevican was 0.772 with an accuracy of 71.70%;for N-Brevican the AUC was 0.849 with an accuracy of 79.20%;for Brevican-A the AUC was 0.853 with an accuracy of 80.00%;and for the N-Brevican/Brevican-A ratio the AUC was 0.940 with an accuracy of 90.00%.Conclusion Brevican-related fragments may serve as potential serum biomarkers for distinguishing AD from VD,with the N-Brevican/Brevican-A ratio showing superior discriminative performance.关键词
