Abstract
Objective To analyze the associations of serum parathyroid hormone(PTH),albumin(ALB),alkaline phosphatase(ALP),and the calcium-phosphorus product with secondary hyperparathyroidism(SHPT)in patients with chronic renal failure(CRF).Methods This retrospective study reviewed clinical data of 80 patients with CRF treated at Jiyuan People's Hospital from January 2022 to November 2024.All patients were followed for 3 months via outpatient visits or telephone.According to the occurrence of SHPT,patients were divided into an SHPT group(n=25)and a non-SHPT group(n=55).General characteristics and laboratory indicators were compared between groups,including serum creatinine,serum uric acid,blood urea nitrogen,serum PTH,serum ALB,serum ALP,serum calcium,serum phosphorus,and the calcium-phosphorus product.A binary logistic regression model was used to analyze risk factors for SHPT in patients with CRF.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of serum PTH,ALB,ALP,and the calcium-phosphorus product for SHPT in patients with CRF.Results Serum PTH,ALB,ALP,serum phosphorus,and the calcium-phosphorus product were higher in the SHPT group than in the non-SHPT group(P＜0.05).Regression analysis showed that higher levels of serum PTH,ALB,ALP,and the calcium-phosphorus product were closely associated with the occurrence of SHPT in patients with CRF(P＜0.05).ROC analysis showed areas under the curve(AUCs)for serum PTH,ALB,ALP,the calcium-phosphorus product,and their combination of 0.884,0.758,0.873,0.867,and 0.917,respectively;sensitivities of 80.00%,72.00%,80.00%,84.00%,and 96.00%,respectively;and specificities of 92.73%,74.55%,83.64%,83.64%,and 74.55%,respectively.All predictive performances were statistically significant(P＜0.05).Conclusion In patients with CRF,SHPT is associated with the levels of serum PTH,ALB,ALP,and the calcium-phosphorus product.Combined measurement of these indicators may help preliminarily assess the risk of SHPT in patients with CRF;however,comprehensive judgment should also incorporate clinical presentation and other auxiliary examinations.关键词
