Abstract
Objective To explore the urinary tract infection(UTI)status and pathogenic bacteria characteristics after TURP and clinical significance of pulmonary surfactant protein A/D(SP-A/SP-D)detection in urine.Methods From January 2020 to January 2024,150 patients with benign prostatic hyperplasia were collected as study subjects and were treated with TURP in Fengqiu County People's Hospital.According to the occurrence of postoperative UTI,the patients were divided into infection group(23 cases)and non-infection group(127 cases).The characteristics of infection were statistically analyzed.The diagnostic value of urine SP-A and SP-D levels on UTI after TURP was analyzed,and the risk factors affecting postoperative UTI were analyzed.Results The postoperative UTI rate of 150 patients with benign prostatic hyperplasia treated by TURP was 15.33%(23 cases).A total of 25 strains of pathogenic bacteria were detected,and Escherichia coli and Klebsiella pneumoniae were the main pathogenic bacteria and accounted for 20.00%(5 strains)and 20.00%(5 strains),and their drug resistance rates to ticarcillin and ampicillin were high(80.00%,80.00%;100.00%,100.00%),and the drug resistance rates to imipenem were low(0.00%,0.00%).The levels of urine SP-A and SP-D in infection group were higher than those in non-infection group(P<0.05).The AUC of combined detection in the diagnosis of UTI after TURP was greater than that of single detection of urine SP-A or SP-D(P<0.05).According to the International prostate symptom score(IPSS),the patients with different degrees of LUTS were divided into mild group(14 cases)and moderate-to-severe group(9 cases).The levels of urine SP-A and SP-D in moderate-to-severe group were higher than those in mild group(P<0.05).The proportions of patients with diabetes mellitus,postoperative catheterization,SP-A≥49.65 pg/mL and SP-D≥101.37 pg/mL in infection group were higher while the proportion of patients with preoperative prophylactic use of antibiotics was lower than that in non-infection group(P<0.05).Postoperative catheterization and SP-A level≥49.65 pg/mL were risk factors for UTI after TURP(P<0.05).Conclusion The main pathogenic bacteria of UTI after TURP in patients with benign prostatic hyperplasia treated by TURP are Escherichia coli and Klebsiella pneumoniae.The abnormal increase of urine SP-A level is a risk factor for postoperative UTI,and the combined detection of various indicators has diagnostic value on the occurrence of postoperative infection.关键词
