精准抗血小板联合脑超声波治疗AIS合并脑微出血的临床效果
Clinical Efficacy of Precision Antiplatelet Therapy Combined with Brain Ultrasonic Wave for the Treatment of Acute Ischemic Stroke Complicated with Cerebral Microbleeds

Abstract
Objective:To explore the clinical efficacy and safety of precise antiplatelet therapy combined with brain ultrasonic wave in the treatment of acute ischemic stroke(AIS)patients complicated with cerebral microbleeds.Method:A total of 150 patients diagnosed with AIS complicated with cerebral microbleeds in the Neurology Department of Xinyu Iron and Steel Group Co.,Ltd.Central Hospital from October 2022 to March 2024 were selected and divided into control group(n=50),ultrasonic wave group(n=50),and observation group(n=50)according to the random number allocation principle.The control group received random antiplatelet treatment,while the ultrasonic wave group received random antiplatelet and brain ultrasonic wave treatment,the observation group received precise antiplatelet treatment combined with brain ultrasonic wave treatment after gene testing for platelet endothelial cell adhesion molecule-1(PEAR1),glycoprotein Ib alpha(GP1BA),and cytochrome P450 2C19(CYP2C19).The treatment course lasted for 3 months of the three groups of patients.The total effective rate,early neurological function deterioration rate,neurological function improvement rate,safety profile,all-cause mortality rate,and stroke recurrence rate among the three groups were compared.Result:The rate of early neurological function deterioration in the observation group was lower than that in the ultrasonic wave group and the control group(P<0.05).After 2 weeks of treatment,the total effective rate of the observation group was higher than that of the ultrasonic wave group and the control group(P<0.05).After 2 weeks of treatment,the incidences of hemorrhagic transformation,systemic hemorrhage and thrombocytopenia in the observation group were all lower than those in the ultrasonic wave group and the control group(P<0.05).After 3 months of treatment,the improvement rate of neurological function in the observation group was higher than those in the ultrasonic wave group and the control group(P<0.05).The follow-up results showed that the recurrence rate of stroke and all-cause mortality in the observation group were both lower than those in the ultrasonic wave group and the control group(P<0.05).Conclusion:For patients with AIS complicated with cerebral microbleeds,precise antiplatelet treatment combined with brain ultrasonic wave treatment demonstrates superior clinical efficacy and good safety compared with either monotherapy of random antiplatelet agents or the combination of random antiplatelet agents and transcranial ultrasonic wave treatment.关键词
