多巴酚丁胺联合硝酸甘油治疗妊娠合并心力衰竭的效果观察
Observation of the Effect of Dobutamine Combined with Nitroglycerin in the Treatment of Heart Failure in Pregnancy

Abstract
Objective:To investigate the effect of Dobutamine combined with Nitroglycerin in the treatment of heart failure in pregnancy.Method:Seventy-five pregnant patients with heart failure who were admitted to the Emergency Department of Ganzhou Women and Children's Health Care Hospital from January 2021 to June 2024 were selected and assigned to the combination group(37 cases)and the Nitroglycerin group(38 cases)using a random number table.The Nitroglycerin group was treated with Nitroglycerin,while the combination group was treated with Dobutamine on the basis of the treatment in Nitroglycerin group.Both groups underwent 1 week of continuous treatment.The clinical efficacy,cardiac function indicators,brain natriuretic peptide,troponin I,disappearance time of clinical symptoms and signs,length of hospital stay,and adverse reactions were compared between the two groups.Result:The total effective rates in the combination group and the Nitroglycerin group were 94.59%and 84.21%,without statistically significant difference(P>0.05).After treatment,left ventricular ejection fraction(LVEF)in the combination group was higher than that in the Nitroglycerin group(P<0.05).Both groups showed decreases in left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD),LVEDD in the combination group was smaller than that in the Nitroglycerin group(P<0.05),but there was no statistically significant difference in LVESD between the groups(P>0.05).The levels of brain natriuretic peptide and troponin I in the combination group were lower than those in the Nitroglycerin group(P<0.05).The disappearance time of shortness of breath in the combination group was earlier than that in the Nitroglycerin group(P<0.05).However,there were no statistically significant differences in the disappearance time of lung sounds,liver retraction time,or hospitalization time between the groups(P>0.05).The incidence rates of adverse reactions in the combination group and the Nitroglycerin group were 13.51%and 7.89%,without statistically significant difference(P>0.05).Conclusion:Combined use of Dobutamine and Nitroglycerin can improve cardiac function and lower the levels of brain natriuretic peptide and troponin I in pregnant patients with heart failure to a certain extent.关键词
