Abstract
Objective:To explore the application value of combined detection of high-risk human papillomavirus(HR-HPV)detection,thin-prep cytology test(TCT),and p16/Ki-67 immunocytochemical double staining(p16/Ki-67 double staining)in cervical tissue in the screening of precancerous lesions of the cervix.Method:A retrospective analysis was conducted on the clinical data of 126 patients with suspected cervical precancerous lesions who needed cervical cancer screening and were admitted to the First People's Hospital of Fuzhou City from January 2022 to December 2024.The histopathological,HR-HPV,TCT and p16/Ki-67 double staining test results of all patients were compared.According to the pathological results of cervical biopsy,the patients were divided into the low-grade lesion group[inflammatory lesions of cervical tissue and grade 1 cervical intraepithelial neoplasia(CIN)lesions]and the high-grade lesion group(grade 2 and 3 CIN lesions and cervical cancer),and the diagnostic value of each method alone or in combination for high-grade cervical cancer lesions was evaluated.Result:Among the 126 patients detected by histopathological examination,61 cases had low-grade lesions and 65 cases had high-grade lesions.The HR-HPV test was positive in 79 cases,among which 46 cases were positive for HPV16/18 and 33 cases were positive for the other 12 types of HPV;TCT detected 60 positive cases;56 positive cases were detected by p16/Ki-67 double staining.With the aggravation of the lesion degree of cervical biopsy,the positive rates of TCT and p16/Ki-67 double staining gradually increased(P<0.05),but there was no statistically significant difference in the positive rate of HR-HPV among different degrees of cervical lesions(P>0.05).The positive rates of HR-HPV,TCT and p16/Ki-67 double staining in the high-grade lesion group were higher than those in the low-grade lesion group(P<0.05).Taking histopathological examination as the gold standard,the combined detection of the three items has the highest diagnostic efficacy for high-grade cervical cancer lesions,with a sensitivity of 86.15%,a specificity of 93.44%,an accuracy rate of 89.68%,and a Kappa value of 0.794.Conclusion:The combined detection of HR-HPV,TCT and p16/Ki-67 double staining in cervical tissue can identify patients with potential high-grade lesions and improve the detection rate of cervical lesions.关键词
