全剧情一体化模拟教学在临床技能培训中的应用研究
Research on the application of integrated scenario-based simulation training in clinical skills education

Abstract
Objective To propose and evaluate the feasibility and advantages of the Integrated Scenario-based Simulation Training(ISST)mode,which integrates AI-assisted history-taking,high-fidelity simulated physical examination,and virtual surgery,over traditional simulation training(modular approach using standardized patients[SPs]and static simulators)in surgical education.Methods A prospective cohort study was conducted with 50 fourth-year medical students,randomly assigned to either the experimental group(n=25)or the control group(n=25).The experimental group underwent continuous,scenario-based training for acute appendicitis,including AI-driven pa-tient interviews,simulated physical examinations with dynamic feedback,and virtual laparoscopic sur-gery.The control group received traditional modular training using SPs,static simulators,and simu-lated surgery.Primary outcomes included clinical decision-making time,OSCE scores,and diagnos-tic accuracy.Results Compared with the control group,the experimental group demonstrated 40.0%shorter clinical decision-making time[(22.7±1.2)min vs.(25.3±2.1)min,P<0.001],higher diag-nostic accuracy(96.0%vs.80.0%,P=0.024),faster recognition of key symptoms and improved physical examination precision.The experimental group achieved better overall performance than the control group.Conclusion The ISST mode significantly enhances clinical competency by integrating cog-nitive,decision-making,and technical skills into a standardized,reproducible,and cost-effective training environment,effectively addressing limitations of traditional methods such as SP variability and frag-mented learning.Further improvements in AI interactivity and haptic feedback could optimize its realism.关键词
