不同经验水平住培医师在3D腹腔镜模拟训练中的适应性与学习效率差异研究
Study on the differences in adaptability and learning efficiency of residents with different experience levels in 3D laparoscopic simulation training

Abstract
Objective To explore the differences in learning efficiency and adaptability of surgi-cal residents with different experience levels in 3D laparoscopic simulation training,and provide a ba-sis for stratified teaching strategies.Methods A total of 32 surgical residents who were trained in our hospital from September 2024 to April 2025 were selected and divided into the novice group(n=18)and the advanced group(n=14)according to whether they had learned about laparoscopy.The two groups uniformly performed four 3D laparoscopic simulation training tasks,and each training was re-peated 3 times.The operation time,subjective score,and self-assessed confidence index were re-corded to further analyze the differences in learning efficiency and adaptation speed of each group.Results Both groups demonstrated progressive skill improvement across four 3D laparoscopic simula-tion tasks.The novice group had longer initial operation times[e.g.,ring placement:(121.7±6.6)s vs.(107.9±7.3)s,P<0.001]but showed greater improvement[(22.9±6.0)s vs.(13.8±5.8)s,P<0.001].In the suturing and knot-tying task,their reduction was nearly twice that of the advanced group[(84.4±15.1)s vs.(49.1±17.3)s,P<0.001).Regarding subjective GRS scores,the advanced group scored higher in the first trial[e.g.,ring placement:(13.4±1.0)vs.(11.8±1.0),P<0.001],but the novice group achieved greater improvement[(3.4±1.0)vs.(2.1±1.0),P=0.001].Similar advan-tages in score increments were observed for needle passing,ball placement,and suturing tasks(all P<0.05).Furthermore,the novice group reported higher median confidence after training(5.0 vs.4.5,P=0.014),while overall satisfaction did not differ significantly between groups(P>0.05).Conclusion 3D laparoscopic simulation training has a good teaching effect on residents with different experience levels.Novice trainees have stronger adaptability and more significant benefits.It is recom-mended that the 3D training system be promoted as a key teaching method in the early stage of resi-dency training.关键词
