柴黄清胰活血颗粒对重症急性胰腺炎大鼠肠道动力障碍的作用及机制
Mechanisms and Effects of Chaihuang Qingyi Huoxue Granules Ameliorating Intestinal Motility Dysfunction in Rats with Severe Acute Pancreatitis

Abstract
Objective To investigate the effects and mechanisms of Chaihuang Qingyi Huoxue Granules(CQHG)on intestinal motility dysfunction in rats with severe acute pancreatitis(SAP).Methods Forty-eight SD male rats were randomly divided into sham-operated group,model group,and CQHG group,each further subdivided into 12-hour subgroup and 24-hour subgroup(n=8 per subgroup).The SAP model was induced by retrograde injection of 3.5%sodium taurocholate into the biliopancreatic duct.The CQHG group received 2.2 g·kg-1 CQHG via gavage,while the sham-operated group and model group received equivalent volumes of saline every 6 hours.Rats were euthanized at 12-hour and 24-hour post-operation,and tissue samples were collected.One hour before euthanasia,charcoal ink(0.1 mL·kg-1)was administered orally to measure small intestinal propulsion rate.Pancreatic and colonic pathological changes were observed via hematoxylin-eosin(HE)staining.Serum amylase(AMY)activity was measured biochemically.Plasma endotoxin(ET)levels were quantified using the limulus amebocyte lysate assay.Serum levels of tyrosine kinase receptor(c-kit),motilin(MTL),substance P(SP),and vasoactive intestinal peptide(VIP)were detected by ELISA.Immunohistochemistry was used to assess SP,VIP,and c-kit protein expression in colonic tissues.RT-PCR was performed to measure mRNA expression of MTL,SP,VIP,and c-kit in colonic tissues.Results At both 12-hour and 24-hour timepoints,compared with the sham-operated group,the model group exhibited significant abdominal ascites,abscess-like changes in pancreatic tissue with saponification spots and extensive necrosis,intestinal distension with gas and fluid accumulation,and adhesions between pancreatic/intestinal tissues and surrounding organs.The small intestinal propulsion rate was significantly decreased(P<0.001).Pancreatic tissue showed swelling,structural disorganization,hemorrhage,and significantly elevated pathological scores(P<0.001).Colonic tissues displayed mucosal damage,inflammatory cell infiltration,congestion,edema,and significantly increased pathological scores(P<0.001).Serum AMY activity and plasma ET levels were significantly elevated(P<0.001).Serum c-kit and MTL levels were significantly decreased(P<0.01,P<0.001),while serum SP and VIP levels were significantly increased(P<0.01,P<0.001).Colonic c-kit protein expression was significantly reduced(P<0.001),while SP and VIP protein expression were significantly increased(P<0.001).Colonic c-kit and MTL mRNA expression were significantly downregulated(P<0.001),whereas SP and VIP mRNA expression were significantly upregulated(P<0.001).Compared with the model group,the CQHG group showed milder pancreatic and intestinal lesions,reduced necrosis,significantly increased small intestinal propulsion rate(P<0.001),alleviated pancreatic acinar swelling and structural disorganization,reduced hemorrhage and necrosis,and significantly decreased pancreatic pathological scores(P<0.001).Colonic tissues exhibited reduced inflammatory cell infiltration,mitigated mucosal damage,congestion,and edema,with significantly lower pathological scores(P<0.001).Serum AMY activity and plasma ET levels were significantly reduced(P<0.01,P<0.001).Serum c-kit and MTL levels were significantly increased(P<0.01,P<0.001),while serum SP and VIP levels were significantly decreased(P<0.01,P<0.001).Colonic c-kit protein expression was significantly upregulated(P<0.001),while SP and VIP protein expression were significantly downregulated(P<0.01,P<0.001).Colonic c-kit and MTL mRNA expression were significantly increased(P<0.001),while SP and VIP mRNA expression were significantly decreased(P<0.001).Conclusion CQHG may ameliorate intestinal motility dysfunction and mitigate pathological damage in pancreatic and colonic tissues in SAP rats by regulating the expression levels of gastro-intestinal-pancreatic endocrine-related hormones.关键词
