重庆市九龙坡区新报告HIV感染者/AIDS患者分子传播网络及治疗前耐药特征分析
Analysis of molecular transmission networks and pretreatment drug resistance in newly reported HIV/AIDS cases in Jiulongpo district,Chongqing

Abstract
Objective To understand the molecular transmission network characteristics of newly reported HIV/AIDS cases in Jiulongpo District,Chongqing,and provide references for targeted AIDS interventions.Methods Newly reported HIV/AIDS cases in Jiulongpo District(2022-2024)were enrolled.Blood samples and demographic data prior to antiviral therapy were collected.After extraction,pol region amplification,sequencing,and sequence alignment,Gene sequences were ana-lyzed for viral subtypes and resistance.The HIV-TRACE tool was used to construct and analyze molecular transmission net-works.Results A total of 222 valid gene sequences were obtained:CRF07_BC subtype(53.60%,119/222),CRF08_BC subtype(24.77%,55/222),CRF01_AE subtype(13.96%,31/222),and other subtypes(7.66%,17/222).Using a 1.5%genetic distance threshold to establish the molecular transmission network,the network entry rate was 37.84%(84/222),forming 31 transmission clusters.The largest cluster comprised 14 males aged ≥50 years.Significant differences in network entry rates were observed across subtypes,age,education,occupation,marital status,and transmission routes(P<0.05).Multivariate analysis showed age ≥50 years(OR=3.303,95%CI:1.363-8.006)and heterosexual trans-mission(OR=4.656,95%CI:1.249-17.347)were associated with higher clustering risk as compared with age<50 years and homosexual transmission.Pretreatment drug resistance rate among these participants was 5.86%(13/222).Con-clusion CRF07_BC,CRF08_BC,and CRF01_AE subtypes are the dominant HIV subtypes in Jiulongpo District.Integra-ting molecular transmission networks with epidemiological field investigation is conducive to identifying outbreak linkages and population/regional clusters.Sustained molecular transmission network surveillance and targeted interventions are es-sential for curbing HIV transmission.关键词
