活动性肺结核患者血清白细胞介素表达谱及泛免疫炎症指标与预后的关系
Correlations between serum interleukin expression profile and pan-immune inflammatory indicators and prognosis in patients with active pulmonary tuberculosis

Abstract
Objective To explore the relationships between the serum interleukin(IL)expression profile,pan-im-mune inflammation indices,and prognosis in patients with active pulmonary tuberculosis(PTB).Methods Totally 204 patients with active PTB were selected.All patients received a 2-month intensive treatment regimen consisting of isonia-zid,rifampicin,pyrazinamide,and ethambutol,followed by a 10-month consolidation treatment with two core anti-tuber-culosis drugs,isoniazid and rifampicin.The prognosis of the patients was evaluated.Relevant prognostic data were collect-ed.Flow cytometry was used to detect the pre-treatment serum IL expression profile,including IL-1β,IL-2,IL-4,IL-6,IL-10,IL-12,and IL-17.An automated hematology analyzer was employed to measure the pre-treatment pan-immune in-flammatory indicators lymphocyte count,neutrophil count,monocyte count,platelet count,C-reactive protein,and albu-min levels.The pan-immune inflammation index(PIV),neutrophil-to-lymphocyte ratio(NLR),monocyte-to-lymphocyte ratio(MLR),and C-reactive protein-to-albumin ratio(CAR)were calculated.Univariate and multivariate Logistic regres-sion analyses were conducted to assess the impact of the serum IL expression profile and pan-immune inflammation indices on the prognosis of patients with active PTB.Results Among the 204 patients with active PTB,136 had a favorable prog-nosis and 68 had an unfavorable prognosis.Compared with patients with a favorable prognosis,those with an unfavorable prognosis had higher proportions of comorbid diabetes,smoking history,fever,and cavitary lesions(all P<0.05),as well as higher serum levels of IL-1β,IL-4,IL-6,IL-12,and PIV(all P<0.05).Elevated serum levels of IL-1β(OR=1.080,95%CI:1.027-1.137),IL-4(OR=1.057,95%CI:1.013-1.103),IL-6(OR=1.089,95%CI:1.039-1.142),IL-12(OR=1.020,95%CI:1.007-1.033),and PIV(OR=1.001,95%CI:1.001-1.002)were risk factors for the prognosis of patients with active PTB(all P<0.05).Conclusion Patients with active PTB who have elevated serum levels of IL-1β,IL-4,IL-6,IL-12,and PIV are more likely to have an unfavorable prognosis.关键词
