从"血不利则为水"探讨膜性肾病病机与辨治
Exploration of Pathogenesis,Syndrome Differentiation and Treatment of Membranous Nephropathy from Perspective of"Impaired Blood Circulation Leading to Water Retention"

Abstract
Membranous nephropathy(MN)is frequently characterized by hypercoagulability as a key pathological feature and edema as the primary clinical manifestation.The associated traits of slowed blood flow and increased thrombotic tendency under hypercoagulable conditions align with the pathogenesis described in traditional Chinese medicine(TCM)as"impaired blood circulation",which denotes a propensity for stasis and stagnation.This paper explores the relationship between hypercoagulability and edema in MN through the TCM principle"impaired blood circulation leads to water retention".We propose that the mutual binding of water and blood stasis constitutes the core pathogenesis underlying MN's development.In treatment,the methods of"invigorating spleen and tonifying kidney to nourish blood and water,removing blood stasis and dredging collaterals to activate blood circulation,and dredging triple energizer to promote diuresis"are adopted to simultaneously regulate blood and water,taking into account both strengthening healthy qi and expelling pathogenic factors,so as to achieve the purpose of activating blood circulation and promoting diuresis without damaging yin and without causing bleeding,provid-ing novel perspectives for TCM-based MN prevention and treatment.关键词
