复方利多卡因乳膏在肿瘤患儿输液港无损伤针穿刺的应用效果观察
Effect of Compound Lidocaine Cream on Non-invasive Needle Puncture of an Implantable Venous Access Port in Children with Cancer

Abstract
Objective:To investigate the efficacy of compound lidocaine cream in alleviating pain during non-coring nee-dle puncture of an implantable venous access port in children with cancer.Methods:A self-controlled,before-after study was conducted and children with malignant tumors were recruited from Sichuan Cancer Hospital by convenience sampling.The intervention was conducted during the third chemotherapy cycle following the implantation of the totally implantable ve-nous-access port(TIVAP),involving the application of compound lidocaine cream one hour prior to non-invasive needle puncture.The Wilcoxon rank-sum test was used to compare outcomes before and after the intervention.The assessed measures included the children's pain levels,scores on the Comfort Behavior Scale,scores on the Children's Medical Fear Scale,the number of successful puncture attempts,and the satisfaction levels of both the children and their families with the pain management during the procedure.Results:A to-tal of 48 children with cancer were included in our study.There were statistically significant differences in the Faces Pain Scale-Revised score(Z=6.068,P＜0.001),comfortable behavior(Z=4.987,P＜0.001),fear of the medical envi-ronment(Z=2.113,P=0.035),self-fear(Z=4.545,P＜0.001),fear of medical operation(Z=5.320,P＜0.001),total score of medical fear(Z=5.442,P＜0.001),and satisfaction of the the child and his/her family members(Z=5.851,P＜0.001)before and after the intervention.Conclusion:The application of compound lidocaine cream sig-nificantly reduced pain,enhanced comfort,alleviated medical fear,and consequently improved the overall medical experi-ence and satisfaction for pediatric cancer patients undergoing TIVAP access.Therefore,its clinical adoption is recommen-ded.关键词
