Meta-analysis of Endometrial Receptivity in Patients with Repeated Implantation Failure Due to Thin Endometrium Treated by Acupuncture and Moxibustion

Abstract
Objective To systematically evaluate the effects of acupuncture and moxibustion on endometrial re-ceptivity(ER)in the patients with repeated implantation failure(RIF)due to thin endometrium.Methods Domestic and foreign databases such as ClinicalTrails.gov,Wanfang Data,Medline,Embase,China National Knowledge Infrastructure(CNKI),and PubMed were searched by computer for the screening of randomized controlled trials on acupuncture and moxibustion for the patients with RIF due to thin endometrium,and manual bibliographic search was conducted simultane-ously,with the time interval from inception to April 2024.According to the inclusion and exclusion criteria,the articles were selected and their quality was evaluated by the Jadad scoring method.The patients treated with acupuncture and moxi-bustion were included as the observation group,and those treated with conventional Western medicine were included as the control group.The clinical pregnancy rate,endometrial thickness,and endometrial blood flow parameters[(resistance index(RI)and pulsation index(PI)]were selected as effect indicators.Meta-analysis was performed in RevMan5.3 software.Results A total of 16 RCTs(1 372 patients)were included,and high-quality articles with the Jadad score ≥4 accounted for 64.71％(11/17).The results of meta-analysis showed that compared with the control group,the observation group had increases in clinical pregnancy rate(OR=2.62,95％CI:2.03-3.38,Z=7.43,P＜0.000 01)and endometrial thick-ness(SMD=1.56,95％CI:0.96-2.16,Z=5.11,P＜0.000 01)and decreases in RI(SMD=-1.04,95％CI:-1.57 to-0.52,Z=3.88,P=0.000 1)and PI(SMD=-1.75,95％CI:-2.64 to-0.85,Z=3.82,P=0.000 1).Conclusion Acupuncture and moxibustion can significantly improve the ER of patients with RIF due to thin endometrium and increase the clinical pregnancy rate to 46.97％through the triple synergistic mechanism of improving uterine artery blood flow,pro-moting endometrial thickening and the expression of receptor molecules,and regulating the synchronization of HPO axis,es-pecially for the patients ≥35 years old and patients after tumor surgery.However,due to the high heterogeneity in some outcome indicators,the conclusions should be interpreted with caution,and future studies should further standardize inter-ventions and measurement methods.关键词
