国医大师雷忠义从痰瘀水停论治扩张型心肌病经验探析
Master of Traditional Chinese Medicine LEI Zhong-yi's Experience in the Treat-ment of Dilated Cardiomyopathy Based on the Theories of Phlegm,Blood Stasis,and Fluid Retention

Abstract
Dilated cardiomyopathy(DCM)is a myocardial disease characterized primarily by ventricular dilation and systolic dysfunction.In modern medicine,treatment mainly involves pharmacological management or heart transplanta-tion.However,these approaches often yield limited efficacy and are associated with significant side effects.Based on tradi-tional Chinese medical(TCM)theory,TCM Master LEI Zhong-yi proposed treating DCM from the perspective of"phlegm,blood stasis,and fluid retention",and established the therapeutic principle of"invigorating qi and warming yang,resolving phlegm,activating blood circulation,and promoting diuresis".At the pathogenesis level,Master LEI emphasized the concept of"deficiency in origin and excess in manifestation",in which the decline of heart qi and yang constitutes the root cause,while the obstruction of the meridians by phlegm and blood stasis leads to internal retention of fluid,forming a vicious cy-cle.In treatment,the self-formulated prescription Yangxin Huoxue Decoction(containing Pseudostellariae Radix,Salviae Miltiorrhizae Radix et Rhizoma,Citri Reticulatae Pericarpium,Schisandrae Chinensis Fructus,Ophiopogonis Radix,Notog-inseng Radix et Rhizoma powder,etc.)serves as the core formula,integrating the therapeutic effects of invigorating qi and warming yang,promoting blood circulation,resolving phlegm,and inducing diuresis.Classic prescriptions such as Tingli Dazao Xiefei Decoction and Zhenwu Decoction are also flexibly modified according to the patient's condition,embodying the principle of"treating the manifestation by eliminating water retention in acute cases and strengthening the body's healthy qi in chronic cases".关键词
