点阵CO2激光联合复方倍他米松注射及医用硅酮凝胶外涂治疗增生性瘢痕
Combined Therapy of Fractional CO2 Laser,Compound Betamethasone Injection and Topical Medical Silicone Gel for Hypertrophic Scars
摘要
Abstract
Objective To explore the clinical effect of combination of fractional CO2 laser and multiple measures in the treatment of patients with post-traumatic hypertrophic scar(HS).Methods Totally 120 patients with post-traumatic HS in the hospital from January 2022 to December 2023 were prospectively selected,and were divided into three groups by adopting simple randomization method according to the random number table method.40 patients in the control group received multiple measures,and 40 patients in the laser group carried out fractional CO2 laser therapy,and the remaining 40 patients in the combined group were given compound betamethasone injection and topical medical silicone gel combined with fractional CO2 laser therapy.After 6 months of continuous treatment,the clinical efficacy,scar aesthetic status[Patient and Observer Scar Assessment Scale(POSAS)],skin status-related factors[type Ⅰ collagen(Col Ⅰ),transforming growth factor β1(TGF-β1),epidermal growth factor(EGF)],aesthetic satisfaction and occurrence of adverse reactions were compared among the three groups.Results After 6 months of treatment,the total effective rate of treatment and aesthetic satisfaction revealed statistical differences among the three groups(P＜0.05).The total clinical effective rate and aesthetic satisfaction om the combined group with 90.00%and 95.00%were higher than 70.00%and 75.00%in the control group(P＜0.05).Compared with the control group,the POSAS scores and levels of Col Ⅰ,TGF-β1 and EGF were lower in the combined group(P＜0.05).The incidence rates of adverse reactions in the combined group,the laser group and the control group were 10.00%,7.50%and 5.00%respectively(P＞0.05).Conclusion On the basis of compound betamethasone injection and topical medical silicone gel,combination of fractional CO2 laser therapy can effectively alleviate the scar conditions,improve the skin status,and enhance the patient satisfaction,and it has good safety.关键词
