Evaluation of the Rational Use of PARP Inhibitors Based on the Entropy Weight Method Combined with TOPSIS Method

Abstract
Objective To evaluate the rational use of poly(adenosine diphosphate-ribose)polymerase(PARP)inhibitors,and to promote their clinical rational use.Methods Based on the drug package insert of Olaparib Tablets,Niraparib Tosilate Capsules,and Fuzolediparib Capsules,combined with relevant guidelines and expert consensus,the drug utilization evaluation(DUE)criteria for PARP inhibitors was established.A total of 132 medical records of patients using PARP inhibitors in the Prescription Automatic Screening System(PASS)of the hospital from January 2023 to September 2024 were selected,with 66 cases containing Niraparib Tosilate Capsules,50 cases containing Fuzolediparib Capsules,and 16 cases containing Olaparib Tablets.The entropy weight method combined with the Technique for Order Preference by Similarity to Ideal Solution(TOPSIS)method was used to evaluate the rationality of PARP inhibitor use.Results A DUE criteria for PARP inhibitors was established,which included three primary indicators and ten secondary indicators.The top three indicators in terms of relative weight coefficients were usage and dosage(37.16%),indications(25.09%),and adverse drug reaction(ADR)monitoring and management(15.30%).In the evaluation of the rationality of drug use,eighty-two cases(62.12%)were completely reasonable(Ci=1.0),six cases(4.55%)were reasonable(0.8≤Ci<1.0),nineteen cases(14.39%)were basically reasonable(0.6≤Ci<0.8),and twenty-five cases(18.94%)were unreasonable(Ci<60%).There were twenty cases(30.30%),three cases(18.75%),and two cases(4.00%)of irrational drug use induced by Niraparib Tosilate Capsules,Olaparib Tablets,and Fuzolediparib Capsules,respectively.The main reasons were irrational usage and dosage,indications,clinical efficacy evaluation,and starting time.Conclusion The DUE criteria for PARP inhibitors established based on the entropy weight method combined with the TOPSIS method is objective and scientific,which can be used for drug rationality evaluation.The application of PARP inhibitors is generally reasonable,but attention still needs to be paid to indicators with relatively high weight coefficients to further reduce medication risks and promote rational clinical use.关键词
