Drug Return Patterns in a Hospital

Abstract
Objective To reduce the return workload,and to ensure the patients'medication safety.Methods A total of 1 122 drug return details from November 2021 to April 2024 were searched through the hospital information system.The reasons for returns were analyzed by different links,administration routes,and return departments,and the returned drugs were classified according to the pharmacological classification items in the MCDEX system.Results The top four reasons for the return of drugs in the hospital were unsalable(38.15%),replacement or discontinuation due to policy reasons(35.38%),damage to the original packaging(9.18%),and near expiration date(6.42%).The top three types of drugs with the highest proportion of returns were anti-infective drugs(16.13%),anti-tumor drugs(13.19%),and cardiovascular system drugs(9.98%).Drugs with oral dosage form had the highest number of returns(557 items),followed by drugs with injectable dosage form(527 items)and drugs with topical dosage form(38 items).There was a significant difference in the proportion of returns among different administration routes,with oral medication of 0.81%,injection medication of 1.45%,and topical medication of 0.40%(P<0.001).The top three departments with the highest proportion of returns were the Department of Internal Medicine Ward Pharmacy(1.20%),Department of Oral Medicine Ward Pharmacy(1.19%),and Department of Surgical Ward Pharmacy(0.80%),with a statistically significant difference(P=0.001).Conclusion The hospital should continuously strengthen drug management,personnel training,and communication among various departments,improve institutional rules,optimize work processes,in order to reduce drug return rates and ensure the quality and safety of drugs.关键词
