Surveying the Cognitive Status of Personnel in Different Levels of Medical Institutions on Hierarchical Diagnosis and Treatment under the Medical Consortium Model

Abstract
Objective To investigate the awareness of graded diagnosis and treatment among physicians from different levels of medical institutions under the medical alliance model,analyze its influencing factors,and provide references for promoting its im-plementation.Methods By distributing a self-made questionnaire within a medical alliance in Chengdu,physicians'knowledge and attitudes toward graded diagnosis and treatment were investigated.Data were processed using SPSS 19.0 software,employing chi-square tests and analysis of variance to evaluate the differences among different groups.Results The results showed that phy-sicians in tertiary medical institutions scored the highest in the knowledge of graded diagnosis and treatment(91.04％),while those in community health service institutions scored the lowest(51.02％).Physicians with over 10 years of work experience had a significantly higher level of awareness compared to those with less than 10 years of experience(OR=3.62,95％CI 2.45,5.35,P<0.001).The influence of professional title was also significant,with senior title physicians scoring72.35±5.02,which was significantly higher than score as 58.19±3.87 of junior title physicians(P<0.001).Conclusion The influence of physi-cians'work experience and professional title on the cognition of hierarchical diagnosis and treatment was significant,while the im-pact of age and educational background was relatively minor.To improve physicians'understanding of hierarchical diagnosis and treatment,it is suggested to strengthen training on hierarchical diagnosis and treatment,optimize the internal cooperation mecha-nism of medical consortium,smooth the"two-way referral"channel,and improve the incentive mechanism for the mobility of medical staff,so as to promote the in-depth implementation of the hierarchical diagnosis and treatment system.关键词
