基于超声衰减参数评估超声联合临床参数模型对肝脏脂肪含量的预测价值
Predictive value of multi-parameter model based on ultrasound attenuation parameters and clinical data for liver fat content

Abstract
Objective:To investigate the predictive value of multi-parameter model based on ultrasound attenuation parameters and clinical data for liver fat content.Methods:A total of 615 people who underwent health examination were selected,and all of them underwent ultrasound examination to measure the visceral fat thickness(VFT)and subcutaneous fat thickness(SFT),and the attenuation coefficient(AC)of ultrasound-guided attenuation parameter(UGAP)were recorded.Among the subjects,68 cases underwent MRI proton density fat fraction(PDFF)examination,and the correlation between MRI-PDFF value and AC value was analyzed.The remaining 547 subjects involved in the prediction model construction and validation,were randomly divided into a training cohort(380 cases)and validation cohort(167 cases)at a ratio of 7∶3.The stepwise regression method was used to construct the prediction model for liver fat content prediction.Pearson correlation and ROC curve was used for analyze the predictive efficiency.Results:UGAP-AC value was highly correlated with MRI-PDFF value(r=0.931).Ultimately,the fatty liver degree diagnosed by ultrasound,VFT,SFT and triglycerides were incorporated into the predictive model.The final constructed multiple linear regression equation was:Logit(AC predicted value)=0.427+0.082×fatty liver degree+0.001×SFT+0.000 388×VFT+0.003×triglycerides.The predicted value of the model was highly correlated with UGAP-AC value(r=0.906).ROC curve showed that the model had the optimal efficacy in diagnosing hepatic steatosis in the validation cohort when the cut-off value was 0.57,with an AUC of 0.985,a sensitivity of 1.00 and a specificity of 0.91.Conclusions:UGAP enables quantitative analysis of liver fat.The predictive value of the model integrating abdominal ultrasound and clinical data demonstrates a strong correlation with UGAP-AC,which can be used for quantitative estimation of liver fat content and identification of populations with hepatic steatosis.This model exhibits favorable clinical utility and excellent diagnostic performance.关键词
