傍针刺治疗血瘀型慢性非特异性下腰痛32例
Clinical effect of proximal needling in treatment of chronic non-specific low back pain with blood stasis:An analysis of 32 cases

Abstract
Objective:To investigate the clinical effect of proximal needling in the treatment of chronic non-specific low back pain with blood stasis.Methods:A total of 64 patients with chronic non-specific low back pain with blood stasis were randomly divided into treatment group and control group,with 32 patients in each group.The patients in the treatment group received proximal needling,and those in the control group received conventional acupuncture,for 30 minutes each time,once a day.Each course of treatment was 7 days,and both groups were trea-ted for 2 courses.The two groups were observed in terms of the changes in Visual Analog Scale(VAS)pain score,Japanese Orthopedic Association(JOA)low back pain rating scale score,Oswestry Disability Index(ODI)score,traditional Chinese medicine(TCM)syndrome score,and the levels of interleukin-6(IL-6)and β-endorphin(β-EP),and clinical outcome was assessed.Results:There was a significant difference in overall response between the treatment group and the control group[93.75％(30/32)vs 87.50％(28/32),P＜0.05].Both groups had significantly better VAS score,JOA score,ODI score,and TCM syndrome score on days 7 and 14 of treatment and 14 days affer treatment,with significantly greater improvements on day 14 and 14 days after treatment,and there were significant differences in these scores between the two groups at the same time point(P＜0.05).After 14 days of treatment,both groups had significantly better expression levels of IL-6 and β-EP,and the treatment group had significantly better levels than the control group(P＜0.05).Conclusion:Compared with conventional acupuncture in the treatment of chronic non-specific low back pain with blood stasis,proximal needling shows a better clinical effect in alleviating lumbar pain and improving dysfunction and blood stasis syndrome,possibly by reducing serum inflammatory factors and increasing the content of endorphin.关键词
