止咳一号方联合糖皮质激素治疗老年重症肺炎痰热壅肺证的临床疗效

Abstract
Objective To investigate the clinical efficacy of Zhike Yihao Formula combined with glucocorticoid in the treatment of elderly severe pneumonia(SP)cases with the syndrome of phlegm-heat obstructing lung.Methods A total of 104 elderly SP patients treated in the Pulmonary Disease Department of Yangzhou Hospital of TCM from December 2020 to December 2023 were enrolled and divided into a treatment group and a control group by referring to the random number table method,with 52 cases in each group.The two groups were given routine interventions according to the relevant SP consensus.The control group was treated with hydrocortisone of glucocorticoid.The treatment group was orally given Zhike Yihao Formula on the basis of the control group.Both groups were treated for two weeks.The therapeutic effect,APACHE Ⅱ score,clinical pulmonary infection score(CPIS),syndrome of phlegm-heat obstructing lung,and serum levels of D-dimer(D-D)and interleukin(IL)-6 were compared between the two groups.Results The total effective rate for the treatment group was evidently higher than that for the control group(P＜0.05).After one or two weeks of treatment,the APACHE-Ⅱ scores of the two groups decreased evidently,and that for the treatment group decreased more evidently(P＜0.01).After one or two weeks of treatment,the CPIS and syndrome of phlegm-heat obstructing lung for both groups decreased evidently,and those for the treatment group decreased more evidently(P＜0.01).After two weeks of treatment,serum levels of D-D and IL-6 in the two groups were evidently decreased,and the reduction for the treatment group was more evident(P＜0.01).Conclusion The clinical efficacy of Zhike Yihao Formula combined with glucocorticoid in the treatment of elderly SP patients with syndrome of phlegm-heat obstructing lung is evident.It is beneficial for the control of pulmonary infection and disease and can reduce serum levels of D-D and IL-6.关键词
