UKA与TKA治疗内侧单间室骨关节炎效果评价
Efficacy of unicompartmental knee arthroplasty versus total knee arthroplasty in treatment of medial compartment osteoarthritis

Abstract
Objective To investigate the efficacy of unicompartmental knee arthroplasty(UKA)versus total knee arthro-plasty(TKA)in the treatment of patients with medial compartment osteoarthritis(MCOA).Methods A total of 120 patients with MCOA who underwent surgical treatment in The Second Affiliated Hospital of Nantong University from January 2017 to Ja-nuary 2019 were enrolled as subjects,and they were divided into control group and observation group using a random number ta-ble,with 60 patients in each group.The patients in the control group underwent TKA,while those in the observation group under-went UKA.The two groups were compared in terms of perioperative indicators,postoperative alignment(including femorotibial angle(FTA),posterior slope angle(PSA),lateral distal femoral angle(LDFA),and medial proximal tibial angle(MPTA)),range of motion(ROM),joint function(HSS score),plantar pressure distribution,and the incidence rate of complications.Re-sults Compared with the control group,the observation group had significantly shorter time of operation,time to voluntary knee flexion to 90°,and length of hospital stay,a significantly lower intraoperative blood loss,and a significantly shorter length of surgi-cal incision(t=5.121-31.400,P＜0.05).There were no significant differences in FTA,PSA,LDFA,and MPTA between the two groups after surgery(P＞0.05).Compared with the control group,the observation group had significantly higher ROM and HSS scores(F=9.579-41.671,P＜0.05),a significantly better plantar pressure distribution(t=2.411-6.333,P＜0.05),and a significantly lower incidence rate of complications at 5 years after surgery(x2=4.183,P＜0.05).Conclusion UKA and TKA have a similar alignment effect in MCOA patients,but UKA shows a significantly better effect in improving ROM,knee joint func-tion,and plantar pressure distribution.In addition,UKA can significantly optimize perioperative indicators,reduce the incidence rate of complications,and enhance surgical safety.关键词
