CT灌注成像对鼻咽癌放疗联合化疗效果的预测价值
Value of CT perfusion imaging in predicting the efficacy of chemoradiotherapy for nasopharyngeal carcinoma

Abstract
Objective To investigate the value of CT perfusion imaging in predicting the efficacy of chemoradiotherapy(CRT)for nasopharyngeal carcinoma(NPC).Methods A retrospective analysis was performed for the clinical data of 125 pa-tients with NPC who attended Tangshan People's Hospital from May 2021 to October 2023.All patients underwent CT perfusion imaging before treatment,and according to the response of patients after CRT,they were divided into response group(complete response(CR)and partial response(PR))and non-response group(progressive disease(PD)and stable disease(SD)).Clinical da-ta and CT perfusion parameters were compared between the two groups.The multivariate logistic regression analysis was used to investigate the influencing factors for the efficacy of CRT,and the receiver operating characteristic(ROC)curve and the area under the ROC curve(AUC)were used to investigate the value of CT perfusion parameters in predicting the efficacy of CRT.Results Among the 125 patients,18 achieved CR,72 achieved PR,32 achieved SD,and 3 achieved PD,yielding an overall response rate of 72.0％.Compared with the non-response group,the response group had a significantly lower proportion of patients with stage T4 NPC,a significantly shorter maximum tumor diameter,and significantly higher blood volume(BV)and blood flow(BF)(x2=4.099,t=2.681-6.764,P＜0.05).The multivariate logistic regression analysis showed that clinical stage,maximum tumor diame-ter,BV,and BF were influencing factors for the efficacy of CRT in NPC(OR=0.370-9.577,95％CI=0.205-2.077 to 0.669-44.157,all P＜0.05).The ROC analysis showed that BV and BF used alone or in combination had an AUC of 0.796,0.837,and 0.917,respectively(95％CI=0.715-0.863 to 0.877-0.970,all P＜0.001)in predicting the efficacy of CRT,suggesting that the combination of BV and BF had the highest predictive value,with a sensitivity of 91.4％and a specificity of 80.0％.Conclusion Clinical stage,maximum tumor diameter,BV,and BF are influencing factors for the efficacy of CRT in NPC.CT perfusion ima-ging parameters have a certain value in predicting the efficacy of CRT in NPC and can provide a reference for developing treatment regimens for NPC.关键词
