Application of Best Evidence in Chloral Hydrate Sedation Management for Pediatric Patients in Oph-thalmology Clinics

Abstract
Objective To develop an evidence-based chloral hydrate sedation management plan for pediatric patients in ophthalmology clinics and evaluate its effects,aiming to provide a reference for developing targeted nursing plans.Methods Based on the"6S"Hierarchy of Evidence-Based Resources,articles regarding chloral hydrate sedation management for pediatric patients in ophthalmology clinics were searched in databases and websites using a top-down approach.Two investigators completed the literature quality evaluation,evidence extraction and summarization.Review indicators were developed based on the best evidence after two rounds of discussions with stakeholders,including a review panel and family representatives of two pediatric patients.From April to June 2022,22 nurses and 78 pediatric patients were selected by convenience sampling from the Ophthalmology Clinic of Eye&ENT Hospital of Fudan University as the subjects for pre-application review.From August to October 2022,the same 22 nurses and 129 pediatric patients were selected by convenience sampling from the same clinic as the subjects for post-application review.The review indicator implementation rate among nurses in the ophthalmic clinic,sedation success rate,sedation time,examination cooperation scores,re-dosing rate and adverse reaction incidence were compared before and after the application of best evidence.Results A total of 9 articles were included,comprising 2 guidelines,1 expert consensus,4 summaries of evidence,1 systematic review and 1 meta-analysis.Finally,24 pieces of best evidence were yielded and translated into 15 quality review indicators.After evidence application,the review indicator implementation rate significantly improved(2 indicators already reached 100%at baseline,13 indicators in total);the sedation success rate in pediatric patients increased from 78.20%to 89.92%(P<0.05);the mean sedation time decreased from 22.00(15.00,35.00)min to 20.00(12.50,30.00)min(P<0.05);the re-dosing rate decreased from 6.41%to 0%(P<0.05).Conclusion The evidence-based chloral hydrate sedation management plan for pediatric patients in ophthalmology clinics can standardize nursing practices,improve sedation efficacy and ensure patient safety.关键词
