基于真实世界评价中药对类风湿关节炎患者免疫球蛋白的影响
Evaluation of the impact of Chinese herbs on immunoglobulins in patients with rheumatoid arthritis based on real-world study

Abstract
Objective:To explore the impact of Chinese herbs on immunoglobulins in patients with rheumatoid arthri-tis(RA)and the prescription patterns in real-world study based on a random walk model.Methods:Medical records of RA inpatients treated in the Rheumatology and Immunology Department of Anhui Traditional Chinese Medicine Hospital from March 2013 to February 2023 were collected.According to whether Chinese herbs were used or not,the baseline character-istics of the two groups of patients were matched 1∶1 using propensity score matching.The patients were divided into a study group(3,902 cases taking Chinese herbs)and a control group(3,902 cases without taking Chinese herbs).Changes in im-munoglobulin levels(IgA,IgG,IgE and IgM),the 28-joint Disease Activity Score(DAS28),and the Visual Analogue Scale(VAS)scores before and after treatment were analyzed in both groups.A random walk and association rule analysis were employed to investigate the impact of Chinese herbs on immunoglobulins and the underlying prescription patterns.Results:(1)After treatment,the levels of IgG,IgE,IgA and IgM,as well as disease activity indicators(DAS28 and VAS scores),decreased in both groups,with more pronounced reductions observed in the study group.The differences were sta-tistically significant(P<0.05,P<0.01).The random walk model revealed a long-term association between the decline in im-munoglobulin levels and the interventions received by patients after treatment.(2)Among the Chinese herbs prescriptions,herbs with spleen-strengthening and dampness-resolving properties,as well as blood-activating and stasis-resolving prop-erties,ranked among the top five in terms of frequency of use.These herbs were Fuling,Honghua,Gancao,Danshen and Chenpi.Cluster analysis identified three clusters,primarily characterized by spleen-strengthening and dampness-resolving,blood-activating and stasis-resolving,and wind-dampness-dispelling effects,respectively.Association rule analysis indicated that Fuling,Honghua,Xixiancao,and Danshen were highly associated with improvements in immuno-globulin levels.Conclusion:In real-world study,Chinese herbs can improve immunoglobulin levels and alleviate symp-toms in RA patients.The herbs Honghua,Danshen,Fuling and Xixiancao exhibit a high association with these improve-ments.关键词
