基于经筋理论探究膝腰联合推拿治疗初中期膝骨关节炎的临床疗效观察
Clinical observation on the efficacy of combined knee-lumbar manipulation based on Meridian Sinew Theory in treating early-middle stage knee osteoarthritis

Abstract
Objective:To investigate the clinical efficacy of combined knee-lumbar manipulation based on Meridian Sinew Theory in treating early-middle stage knee osteoarthritis(KOA).Methods:Eighty KOA patients from The First Af-filiated Hospital of Anhui University of Chinese Medicine between March 2024 and March 2025 were enrolled and ran-domly divided into an observation group(n=40)and a control group(n=40).Patients in the observation group received combined knee-lumbar manipulation,while patients in the control group received only conventional knee manipulation.Both groups were treated for 4 weeks.Serum levels of inflammatory factors(IL-1β,IL-6 and TNF-α),WOMAC index,Visual Analogue Scale(VAS)scores,and surface electromyography(sEMG)signals-including mean power frequency slope(MPFs),median frequency(MF),and average electromyography(AEMG)-were measured in both groups before and after treatment.Clinical efficacy was compared between the two groups.Results:After treatment,serum inflammatory factor levels,WOMAC scores and VAS scores significantly decreased in both groups(P<0.01),with the observation group showing sig-nificantly lower levels than the control group(P<0.05).sEMG results indicated that compared to the control group,the ob-servation group had significantly increased MPFs and AEMG in the vastus medialis(VM)and vastus lateralis(VL)muscles,significantly decreased MPFs in the biceps femoris(BF)and significantly increased AEMG in the BF,demonstrat-ing superior improvement in muscle function(P<0.05).The total effective rate was 95.00%in the observation group,higher than the 80.00%in the control group(P<0.05).Conclusion:Combined knee-lumbar manipulation based on Merid-ian Sinew Theory can significantly reduce inflammatory responses,improve knee joint function and enhance muscle biome-chanical status in KOA patients,demonstrating superior efficacy compared to conventional knee manipulation.This approach provides a new clinical strategy for KOA treatment.关键词
