子宫颈癌调强放射治疗中血液学毒性与骨盆骨髓剂量的相关性分析
Analysis of correlation between hematologic toxicity and dosimetric parameters of pelvic bone marrow in cervical cancer patients undergoing intensity-modulated radiation therapy

Abstract
Objective To study the correlation between acute hematological toxicity and the dosimetric parameters of the whole pelvic bone marrow and its sub-volumes in cervical cancer patients undergoing intensity-modulated radiation therapy(IMRT).Methods The clinical information and dosimetric parameters of 107 cervical cancer patients who received initial radical IMRT at our hospital from Oc-tober 2018 to December 2023 were retrospectively analyzed.The clinical features and dosimetric parameters between patients with dif-ferent grades of acute hematological toxicity were compared.Results Significant differences were observed in International Federation of Gynecology and Obstetrics(FIGO)staging,concurrent chemotherapy status,the V40 and Dmax of the whole pelvis,and the V10,V20,V30,and Dmean of lumbosacral spine between patients with grade 0-2 and grade 3-4 acute hematological toxicity(all P<0.05).The V10 of lumbosacral spine was moderately related to acute hematological toxicity(r=0.523,P<0.01).The receiver operating characteristic curve for predicting grade 3-4 acute hematological toxicity by lumbosacral spine V10 showed that the area under the curve was 0.819,and us-ing a cut-off value of 92.51%,the sensitivity,specificity,and accuracy were 72.2%,88.7%,and 83.2%,respectively.Lumbosacral spine V10>92.51%was an independent risk factor for grade 3-4 acute hematological toxicity(OR=23.451,P<0.01).Conclusions Lumbosacral spine V10>92.51%is an independent risk factor for grade 3-4 acute hematological toxicity in cervical cancer patients with IMRT.关键词
