摘要

Abstract

Objective To explore the mechanism of Zhuanggu Formula(壮骨方,ZGF)intervening in osteoclast differentiation in post‐menopausal osteoporosis(PMOP)rats by regulating macrophage M1/M2 polarization.Methods PMOP rat model was established by removing bilateral ovaries.After successful modeling,the rats were randomly divided into a model group,a positive control group,and low/medium/high dose groups of ZGF.ZGF or Alfacalcidol+Calcium Carbonate D3 intervention were given.At the same time,rats from the same batch were selected for sham surgery treatment as the sham surgery group,and were given equal volume of saline via intra‐gastric administration.After 12 weeks,samples were taken for Micro-CT analysis of quantitative indicators and subtle changes in bone structure and density;Tartrate resistant acid phosphatase(TRAP)was used to detect the activity level of osteoclasts;Immunofluores‐cence assay was used to detect the expression levels of IL-1 β,TNF-α,IL-4,and IL-10 in bone tissue;qPCR and Western blot(WB)were used to detect the expression levels of M1/M2 polarization markers iNOS,MHC-Ⅱ,CD206,and CD163 in macrophages within bone tissue.Results Compared with the sham surgery group,the model group showed a significant decrease in bone volume fraction(BV/TV),trabecular number(Tb.N),and trabecular thickness(Tb.Th)(P<0.01),a significant increase in structural model index(SMI)and trabecular separation(Tb.Sp)(P<0.01),a significant increase in the number of osteoclasts(P<0.01),a significant increase in IL-1β and TNF-α levels(P<0.01),a significant decrease in IL-4 and IL-10 levels(P<0.01),a significant increase in mRNA and protein expression of nitric oxide synthase(iNOS)and histocompatibility complex Ⅱ(MHC-Ⅱ)(P<0.01),and a significant decrease in mRNA and protein expression of CD206 and CD163(P<0.01).Compared with the model group,all groups of ZGF were able to significantly increase BV/TV,Tb.N,and Tb.Th levels(P<0.05),downregulate SMI and Tb.SP levels(P<0.05),reduce osteoclast numbers(P<0.01),downregulate TNF-α,IL-1 β,iNOS,and MHC-Ⅱ levels(P<0.05),and upregulate IL-4,IL-10,CD206,and CD163 levels(P<0.05).Conclusion ZGF can improve bone density and trabecular microstructure in PMOP rats,inhibit osteoclast differentiation,and its mechanism is related to inhibiting polarization of M1 macrophages and promoting polarization of M2 macrophages.
