瑞马唑仑复合丙泊酚-依托咪酯用于老年患者无痛胃肠镜诊疗的麻醉效果
Anesthetic effect of remimazolam combined with propofol-etomidate in the anesthesia for painless gastrointestinal endoscopy in elderly patients

Abstract
Objective To evaluate the efficacy and safety of remimazolam combined with propofol-etomidate for painless gastrointestinal endoscopy in elderly patients.Methods A total of 244 elderly patients undergoing painless gastroenterosco-py were selected as research subjects and randomly divided into remimazolam besylate+propofol-etomidate mixture group(group RA,n=122)and propofol-etomidate mixture group(group C,n=122).The systolic blood pressure(SBP),dias-tolic blood pressure(DBP)and heart rate(HR)of the two groups were compared at different time points:before anesthesia induction(T1),when the gastroscope passed through the larynx(T2),at the start of colonoscopy(T3),when the colono-scope passed through the hepatic flexure(T4),and at the time of awakening(T5).Besides,the onset time,time to leave the room,total dosages of propofol-etomidate,and the incidences of adverse reactions including decreased oxygen saturation(SpO2),myoclonus,hiccups,coughing,body movement,somnolence,injection pain,dizziness,nausea and vomiting,and delirium were also compared between the two groups.Results The decrease in SBP and HR in the group RA was smaller than that in the group C,the onset of anesthesia and duration of action were longer than those in the group C,the total dose of propofol-etomidate was less than that in the group C,and the incidence of muscle tremors and injection pain was lower than that in the group C,and differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of other adverse reactions such as decreased SpO2,hiccup,coughing,body movement,and somnolence between the two groups(P>0.05).Conclusion Based on the analysis of anesthesia effects for painless gastrointestinal endoscopy in the elderly patients,propofol-etomidate mixture combined with appropriate amount of remimazolam can prolong the onset time and exit time,but it shows certain advantages in maintaining patient hemodynamic stability,reducing muscle tremors and the incidence of injection pain.关键词
