改良阻断浅静脉回流装置在下肢深静脉血栓溶栓治疗中的应用研究
Study on application of modified superficial venous reflux occlusion device in thrombolytic therapy for lower extremity deep venous thrombosis

Abstract
Objective To explore the optimal usage protocol of a modified superficial venous reflux occlusion device in thrombolytic therapy for lower extremity deep venous thrombosis(DVT),so as to specify its compression site,inflation volume,and balloon pressure.Methods A total of 60 patients undergoing lower extremity DVT were randomly divided into control group and observation group,with 30 cases in each group.Control group:the compression point of great saphenous vein was 4-5 cm above the ankle joint,and small saphenous vein was 8-10 cm above the ankle joint,with an inflation volume of 12-15 mL and a balloon pressure of 110-120 cmH2O.Observation group:the compression point of the great saphenous vein was 6-8 cm below the knee joint,and the small saphenous vein was the same as the control group,with an inflation volume of 8-10 mL.The degree of deep venous perfusion in the lower limbs(Thrombolysis in Myocardial Infarc-tion[TIMI]grade),superficial venous occlusion efficacy,and patient comfort between the two groups were compared.Results The observation group showed significantly higher rates of deep venous perfusion grade Ⅲ(93.33%),superficial venous occlusion grade 0(80.00%),and the proportion of moderate and high comfort levels(96.67%)were significantly higher than those in the control group(46.67%,26.67%,70.00%),and differences were statistically significant(P<0.001).The balloon pressure in the observation group was significantly lower than that in the control group([70-100 vs.110-120]cmH2O)(P<0.001).Conclusion The optimized protocol for the modified occlusion device is as follows:great saphenous vein compression at 6-8 cm below the knee joint,small saphenous vein compression at 8-10 cm above the ankle,with an inflation volume of 8-10 mL and a balloon pressure of 70-100 cmH2O.This protocol can effectively enhance deep venous perfusion,improve superficial venous occlusion,and increase patient comfort.关键词
